depends how i feel. sometimes i will help that lady and kill the wolves trying to eat her, sometimes i'll shoot her in the face loot her body. then kill the wolves and sell their hides.
Always Good on my first run through then Evil on the second.
Sadly I don't think the games that do have that option aren't all that balanced.
Most of the time picking the evil side will cost you in the long run, losing mission options for instance when you cut off dialog or kill someone who has some minor role later. Many times people just wont talk to you, so I sorta feel like I miss out when being totally evil.
So I end up going good on my first play though almost every time, then go evil on 2nd play though. Then I know who has what to offer, and what I would miss by acting out on someone.
